Welcome to the Sri Lanka Medical Council (SLMC) online application form. In order to apply for membership to SLMC you need to register here with us.

It's a simple process which should take you less than a minute. Once you have registered you will receive an email that will provide you with the link for user login. Once you have logged in please complete the form with accurate information.

• Please note that you need to provide us with your full name, contact details and personal information as it appears on official documents like birth certificate/NIC/passport.
• Please note that it is mandatory to have an email address to apply for registration with the SLMC.

Access to the application system is restricted to SLMC staff, applicants/SLMC registrants and other agents or 3^rd party contractors carrying out work commissioned by the SLMC. By applying here you are agreeing to the terms and conditions of use of our application form and system and the Data Protection Statement.

Data Protection Statement

Your data is securely held and protected by the Sri Lanka Medical Council; any data you enter here will be treated confidentially and will be used only by appropriate staff and agents of SLMC involved in the application processing and approval process. The data given through the application system will not be given to other organizations other than those working for and with the Sri Lanka Medical Council if and when it is needed. However please note that some level of information of all those granted registration by the SLMC will be available to the general public. Please read the General Privacy statement for more information on this.

Terms and conditions

1. The SLMC reserves the right to change the terms and conditions given here.
2. Applicants/registrants are responsible for regularly reviewing these terms and conditions and your continued use of this site constitute your agreement to all such terms and conditions.
3. The use of this Online Application form and site is entirely at your own risk.
4. Although this service of online applications is offered free to applicants, there will be a registration fee for all categories of registration and other services provided by the SLMC.
5. This service of online applications may be terminated at any given time, although there is no such intention.
6. The Online Application system and the SLMC cannot be held responsible for any loss or damage due to the failure or termination of the service and we will make reasonable efforts to notify you of foreseeable service interruptions.
7. It is not intended as a business service (e.g. a helpdesk)
8. You (the applicant) take full responsibility for the accuracy of the details entered in this application form.
9. If any third party site is linked to/from the Online Application form/ site, it does not mean the SLMC endorses, or has any responsibility for the site in question, or anything which appears on it.
